It's simple! Book online via calendar or call our office @ 615-400-7034. You'll get your appointment confirmation via email. The next day we are open we will send our intake forms to you.

Drop off or mail, email or text us a swab or a pic of your swab with your saliva on it (PO Box 222, Franklin, Tn 37065). Place swab in a ziploc bag with your first and last name on the bag. (see video below) If mailing from out of state ensure at least one week for it to arrive at our office prior to your appointment.

We recommend a follow up in 4-8 weeks. We usually see incremental improvement and this is often encouraging during your journey towards healing.
