It's simple! Email or call our office @ 615-400-7034 . That will let us know to look for your swab. If you are a new client, we will be in touch with intake forms.

Drop off or mail us a swab with your saliva on it (PO Box 222, Franklin, Tn 37065). Place swab in a ziploc bag with your first and last name on the bag. (see video below) If mailing from out of state ensure at least one week for it to arrive at our office prior to your appointment. An alternative that is super quick and easy is to email or text us a pic of the swab in the bag with your name.

We recommend a follow up in 4-8 weeks. We usually see incremental improvement and this is often encouraging during your journey towards healing.
